PERSONAL DATA SUBJECT APPLICATION FORM

GENERAL INFORMATION:
Pursuant to Article 20 of the Constitution of the Republic of Turkey, everyone has the right to demand the protection of personal data concerning him/her. This right includes the right to be informed about personal data concerning the person, to access such data, to request their correction or deletion, and to learn whether they are used for their intended purpose.
Law No. 6698 on the Protection of Personal Data ("PDP Law") provides the possibility to make certain requests regarding the processing of personal data.
Aydın Ünlüer Turizm Gıda Ticaret ve Sanayi Anonim Şirketi (Company), in accordance with Article 13 of the PDP Law, the evaluation of the rights of personal data owners arising from the PDP Law and the mandatory information to be provided to personal data owners are carried out through this Personal Data Owner Application Form.
- RIGHT TO APPLY
- Application Subjects
Pursuant to Article 11 of the PDP Law, anyone whose personal data is processed may apply to our Company as detailed below and make requests regarding matters related to him/her-
- Learning whether your personal data are processed,
- Request information if your personal data has been processed,
- Learning the purpose of processing your personal data and whether they are used in accordance with their purpose,
- To know the third parties to whom your personal data is transferred domestically or abroad,
- To request correction of your personal data in case of incomplete or incorrect processing, to request the deletion or destruction of your personal data in accordance with the conditions set out in the Law No. 6698 on the Protection of Personal Data,
- To request correction of your incomplete or incorrectly processed personal data and to request notification of the deletion or destruction of your personal data to third parties to whom personal data has been transferred,
- To object to this result in case a result arises against you by analyzing your processed data exclusively through automated systems,
- In case you suffer damage due to unlawful processing of your personal data, to demand the compensation of the damage
you have rights.

- Situations Excluded from the Scope of the Right to Apply
Pursuant to Article 28 of the PDP Law, personal data subjects will not be able to assert their rights in the following cases-
- Processing of personal data by natural persons within the scope of activities related to themselves or their family members living in the same residence, provided that their personal data is not disclosed to third parties and the obligations regarding data security are complied with,
- Processing of personal data for purposes such as research, planning and statistics by anonymizing them with official statistics,
- Processing of personal data for artistic, historical, literary or scientific purposes or within the scope of freedom of expression, provided that it does not violate national defense, national security, public security, public order, economic security, privacy of private life or personal rights or does not constitute a crime,
- Processing of personal data within the scope of preventive, protective and intelligence activities carried out by public institutions and organizations authorized by law to ensure national defense, national security, public security, public order or economic security,
- Processing of personal data by judicial or enforcement authorities in relation to investigation, prosecution, trial or execution proceedings.
Pursuant to Article 28, paragraph 2 of the PDP Law, it will not be possible for personal data subjects to assert their rights (except for the right to claim damages) -
- Processing of personal data is necessary for the prevention of crime or criminal investigation.
- Processing of personal data made public by the data subject himself/herself.
- Personal data processing is necessary for the execution of supervisory or regulatory duties and disciplinary investigation or prosecution by authorized and authorized public institutions and organizations and professional organizations in the nature of public institutions based on the authority granted by the PDP Law.
- Processing of personal data is necessary for the protection of the economic and financial interests of the State in relation to budgetary, tax and fiscal matters.
- APPLICATION METHOD
You can submit your requests within the scope of your rights listed in Article 11 of the PDP Law, in accordance with Article 13 of the Law and Article 5th of the Communiqué on the Procedures and Principles of Application to the Data Controller, you can submit your application to our Company with this Personal Data Owner Application Form by one of the methods described below.

- PERSONAL DATA OWNER'S RIGHT TO FILE A COMPLAINT TO THE BOARD
Pursuant to Article 14 of the PDP Law, in cases where the application is rejected, the response is found insufficient or the application is not responded to in due time, the personal data owner may file a complaint to the Board within 30 (thirty) days from the date of learning the response of our Company and in any case within 60 (sixty) days from the date of application.
A complaint to the Board will not be possible without an application to our Company.
In line with the above-mentioned requests, I kindly request that my application to your Company be evaluated in accordance with Article 13 of the Law and that I be informed.
